Abstract
Million Hearts is a national initiative to prevent one million heart attacks and strokes and empower Americans to make healthy choices. The National Interprofessional Education and Practice Consortium to Advance Million Hearts was founded by the College of Nursing in 2012, and now has more than 150 partners. The consortium designed an online educational program that has led to cardiovascular screening and education of more than 60,000 people nationwide. The educational program, Million Hearts Fellowship is a teaching tool for health sciences students, and it is designed for community outreach and population health prevention. Exemplars of how to embed the Million Hearts Fellowship into curricula and community outreach initiatives will be detailed.
Description
Million Hearts is a national initiative launched by the Department of Health and Human Services with goals to prevent one million heart attacks and strokes, empower and educate Americans to make healthy choices, and target care by focusing on the "ABCSs." Appropriate aspirin therapy, blood pressure control, cholesterol management, smoking cessation, and stress management or the "ABCSs" are the foundation of the initiative. By focusing on these five modifiable lifestyle behaviors, cardiovascular disease is preventable by 80 percent. This presentation will provide an overview of the Million Hearts® initiative and describe the National Interprofessional Education and Practice Consortium to Advance Million Hearts (NIEPCAMH). NIEPCAMH, founded by Dr.Bernadette Melnyk, is an association of academic institutions, community groups, and health care organizations who are committed to the advancement of Million Hearts® and promoting population health through primary prevention, screening, and early identification of cardiovascular disease. As part of NIEPCAMH, partners participate in an online interprofessional educational program. The four-step program involves individuals watching a series of online lectures, participating in 10 Million Hearts cardiovascular screenings in their communities, entering de-identified information into an online survey on the participants they screened, and taking a post-test. Following successful completion of the post-test, individuals are certified as Million Hearts Fellows. This program has led to the cardiovascular screening and healthy lifestyle education of more than 60,000 people nationwide. The 10-step screening process will be reviewed. The Million Hearts Fellowship module is used as a teaching tool for health sciences' colleges and community initiatives. An overview of how the Million Hearts Fellowship has been integrated into health sciences' curricula and community outreach will be discussed. Several of the community outreach initiatives will be detailed including: community church outreach, the Ohio Nurses Association partnership, and on-campus student heart checks.
