Abstract

We have partnered with Healthy New Albany to integrate their programming with our medical expertise by using state-of-the-art technologies together with primary care, integrative medicine, sports medicine and physical therapy practices. The central theme is health and wellness rather than episodic and reactive care.

Description

A number of factors are necessitating imminent change in American health care. In contrast to the traditional doctor-driven medical model, the Philip Heit Center for Healthy New Albany features physical therapists, dietitians, physicians, and fitness specialists focused on the patient and more broadly, a community-centered approach to improving health. We have partnered with Healthy New Albany to integrate their programming with our medical expertise by using state-of-the-art technologies together with primary care, integrative medicine, sports medicine and physical therapy practices. The central theme is health and wellness rather than episodic and reactive care. We all come together with the goal of becoming the "healthiest community in America." This session will highlight both the approach and the early outcomes of this population health-based approach and its efforts to advance research and education on transformative community health goals around the use of exercise, health coaching and behavior change that are consistent with both the APTA vision and Exercise is Medicine (EIM) global initiative.
