Abstract

The Medication Management Program is an innovative telehealth program located within The Ohio State University College of Pharmacy. Through personal coaching from our pharmacists, pharmacy technicians, and pharmacy interns, we empower patients across the country to take control of their health. We focus on educating millions of patients nationwide on their chronic disease states and how to get the most benefit from their medications. We utilize a software developed by SinfoniaRx to serve our patients.

Description

IMPACT. 1: In 2016, the Medication Management Program completed over 68,000 comprehensive medication reviews with patients nationwide. These interactions involved counseling around medication adherence, safety concerns, effectiveness of treatment, and medication cost. -- 2. Our medication reviews have significant impact on overall health care expenditures. On average, our recommendations save ten dollars in overall healthcare spending for every dollar invested into the program. -- 3. We have trained 155 student pharmacists and 61 undergraduate students. They develop critical patient care skills and learn how to interact with a variety of diverse patient populations.
