Abstract
We propose a study to evaluate the impact of health information exchange between the community pharmacist and the epilepsy clinic. We will recruit 50 patients with epilepsy from our clinic to be in this 6-month pilot study. Patients will be randomized into either the ACTIVE or CONTROL group. The 25 patients randomized to the ACTIVE group will be contacted by the community pharmacist in-between their visits to the epilepsy clinic. The information gathered by the community pharmacist will be shared (exchanged) with the epilepsy clinic. Patients in the CONTROL group will have no additional contact with the community pharmacist in-between their epilepsy clinic visits. At this time, 7 patients have been recruited. The outcome measures from these patients are discussed further in the thesis. The purpose is to study the impact of this information exchange on patient outcomes and evaluate the feasibility of growing this new program.
