Abstract

In a 17-hospital Midwest U.S. healthcare system, staff identified a significant increase in CLABSI rates across several facilities. An investigation was initiated to determine a potential root cause. A review of CLABSI-related policies was completed, and no comprehensive system- wide CLABSI prevention policy was found. Therefore, “In high-risk patients (P), how do system-wide best practices/strategies (I) compared to current practice (C) affect CLABSIs (O)?” was used for a systematic review of the literature to determine the best evidence-based practices for the creation of a system-wide CLABSI prevention policy. This initiative began in October 2024 and concluded in March 2025. The DNP Leader created a policy and plan for the incremental rollout and evaluated the rollout response. CLABSI prevention best practices were deployed to one unit within the system to test the implementation efficacy; it will then be deployed to the rest of the system.
