Knowledge, Attitudes, Behaviors of Nursing Faculty and Students’ About Lesbian, Gay, Bisexual and Transgender People

Nurses should provide culturally competent care to all patients including lesbian, gay, bisexual and transgendered (LGBT) people. Although there is a paucity of research, nurses have been found to be more homophobic and less knowledgeable than other healthcare professionals (HCPs) despite having more patient interaction. Minority stress was the conceptual framework for this study. Stigma, operating through minority stress, contributes to the elevated risk of physical and mental disorders to those who identify as LGBT. This exploratory study was conducted to determine the knowledge, attitudes, and behaviors of a large Midwestern university’s nursing students and faculty toward LGBT people. The survey consisted of 5 parts: Knowledge about Homosexuality Questionnaire (KAH), Index of Attitudes Toward Homophobia (IAH), Homophobic Behavior of Students Scale (HBSS), LGBT phobia scale and demographics tool. The survey was distributed to all undergraduate nursing students after classes and to faculty via their mailboxes. The sample consisted of 36 faculty members and 333 nursing students. The sample ranged from 18 to 69 years of age with most identifying as heterosexual (97%), Caucasian (91%), Christian (75%), and Republican (34%). For the KAH, the mean score for faculty was 70%, and for students 64%. For the IAH, the mean for faculty was 55.4 and for students 63.4 (possible 25-125; higher scores indicate greater homophobic attitudes). For the HBSS, the mean for faculty was 18.2 and for students 27.2 (possible 0-100; higher scores indicate higher homophobia). The results indicate a lack of knowledge regarding the culture of LGBT people and a discrepancy between homophobic attitudes and behavior, which requires further explanation. Homophobia exists among nursing students and faculty. While education could improve knowledge, research is needed to determine how to lessen homophobic attitudes and behavior of HCPs, both current and future, in order to eliminate the stigma surrounding LGBT patients.
